Parents/Guardians can fill out their child’s back-to-school forms, online. The forms are easy to complete and all data is protected and secure. For families with multiple children attending Broward County Public Schools, some information will only need to be entered once.

Forms can be filled out in English, Spanish, Haitian-Creole and Portuguese. within the Back to School Wizard. Student’s identification number are needed to complete these forms at the start of each school year for each student enrolled in the District.

To access the Back to School Wizard, which walks parents through the process of completing Back to School Online forms including The Code of Student Conduct and Discipline Matrix.

Online forms are easy to complete through the Broward County Public Schools, and all data is protected and secure.

School Environmental Safety Incident Reporting


The SESIR System currently collects data on 26 incidents of crime, violence, and disruptive behaviors that occur on school grounds, on school transportation, and at off-campus, school-sponsored events, during any 24-hour period, 365 days per year.

Incidents are reported by schools to the districts which, in turn, provide the data to the DOE. Per F.S. 1002.20, parents of public-school students have a right to access school safety and discipline incidents as reported pursuant to s. 1006.07(9).  To view the state reported discipline data:

School Environmental Safety Incident Reporting data is posted on the Florida Department of Education’s website along with Broward County Public School’s website.
